GREAT NEWS!!!

DSA 2011 CHICAGO Conference registration form is now available online.

 

Print out the form, fill it out, and sign two forms:  "Registration" & “Cancellation / Refund Policy”.  Then mail those two forms with your payment  (checks or credit card) to our registration coordinator.!!  Your cancelled check or credit card statement is your receipt.

Warning: If the credit card was not approved, your registration form will be declined. Your cashier check or money order (no personal check will be accepted) to redo your registration.
